Walk-in Passport Week

Visit the TechZone March 23–27 to apply for your passport. No appointment needed! It’s first-come, first-served, and wait times may vary. Call 330.643.9145 to find out what you need to bring.

The last appointments are at 6 pm Monday - Thursday and 4 pm on Friday. Walk-in passports will not be done on Saturday or Sunday. Call with questions 330.643.9145.

What you need to bring for a successful visit:

• If you are applying for an adult passport:
  • A completed, unsigned passport application (Form DS-11) filled out in black ink.
    • Form DS-11 are available on the U.S. Department of State website. 
  • Proof of U.S. Citizenship (one of the following must be submitted with the application):
    • Certified U.S. Birth Certificate
    • Previous U.S. Passport (may be expired, must be undamaged)
    • Consular Report of Birth Abroad
    • Certificate of Naturalization/Citizenship
  • Proof of Identity
    • Valid Driver's License (plus second ID if issued outside of Ohio)
    • Undamaged U.S. Passport (if issued less than 15 years ago)
    • Valid government ID (city, state, or federal)
    • Valid Military ID
  • A Passport photo (available at the Library during your appointment for a $12 fee plus sales tax)
  • A check or money order that goes to the U.S. Department of State

• If you are applying for a child 15 or under:
  • A completed, unsigned passport application (Form DS-11) filled out in black ink.
  • Proof of U.S. Citizenship (one of the following must be submitted with the application):
    • Certified U.S. Birth Certificate
    • Previous U.S. Passport (may be expired, must be undamaged)
  • Proof of Parental Identity for anyone listed on the child's birth certificate
    • Valid Driver's License (plus second ID if issued outside of Ohio)
    • Undamaged U.S. Passport (if issued less than 15 years ago)
    • Valid government ID (city, state, or federal)
    • Valid Military ID
  • A check or money order that goes to the U.S. Department of State
